Open to all adults age 18 and older in Eastern CT
SERAC will use this data to drive our understanding of the needs and capacity of our region to address substance use, problem gambling, mental health and suicide.
The local prevention council in the community with the highest participation rate (as a percentage of population) will receive a $1000 award.
We encourage you to complete the survey yourself and to encourage others in your community to complete it as well.
